Dear student-athletes,

You now have the opportunity to participate to the continuous improvement of our sport programs, and your team. Please take the time to read each statement carefully and check the answer that most closely corresponds to your opinion on the matter. Additional comments can be provided at the end of each section. This Athlete Satisfaction Questionnaire allows you to provide feedback on the Team Environment, the Team Leadership, your Individual Development and the Integrated Support Team (IST). Answers will remain anonymous and confidential.
